公告內容	臺南市114年「早餐GO-學力營養雙加分校園健康飲食獎勵計畫」活動開跑囉~~ 參加資格：本市國中以上各級學校（國中、高中職、大專院校）在學學生。活動期間：114年8月19日（星期(二)）起至10月10日（星期五）止活動步驟 ✅活動期間拍下自己創意搭配的早餐餐點照片及相關說明資料，完成上傳符合資格者即可獲得1次抽獎機會。 ✅參加者選擇要投件類別：(1)中式早餐、(2)西式早餐、(3)便利商店早餐、(4)臺南小吃，上傳之照片菜色比例須仿照國民健康署「我的餐盤」，可參考口衛生福利部國民健康署「我的餐盤」均衡飲食圖像及訣自行搭配(網址：https://www.hpa.gov.tw/Pages/List.aspx?nodeid=4686) ✅可多次(不同餐點照片)上傳，惟每人累計最多5次抽獎機會，且每人限得獎1次。 ✅上傳餐點與文字說明符合以下任2項原則者即具備抽獎資格：(1)我的餐盤原則(2)減糖、(3)減鹽、(4)攝取全榖雜糧。 ✅於活動期間，將陸續由營養師篩選評比投件照片符合資格者，於10月10日活動結束後，統一進行抽獎。 📌活動上傳指定網址： https://p.tainan.gov.tw//UhacZ2 📌抽獎時間：於10月31日前抽出，並公告至本局官網最新消息。活動獎勵： 🎁早鳥健康獎：114年8月19日徵稿至8月31日止，進行第一梯次抽獎，名額30名各可獲得200元禮券(每人限領1份)，9月30日前公布於本局官網。 🎁健康生活獎：徵稿至10月10日止，所有合格參加者進行抽獎，名額50名各可獲得200元禮券(可與早鳥獎重複得獎)。 🎁健康早餐代言獎：同一參賽者於四種類別皆投稿至少1次，且經本局營養師評比皆合格者，抽出20名800元禮券，另頒發「最佳健康早餐代言人」獎狀一份。 !!!注意事項 ⭐ 參加者須為本市國中以上各級學校在學學生，並提供真實資料以利通知獲獎，未滿20歲之參加者請於google表單加填法定代理人資訊。 ⭐獎項於活動結束後統一寄送，獲獎人須配合填具簽領資料後始能領取禮券。 ⭐主辦單位保留對活動方式、辦法及獎項修改之權利，如遇人力不可抗拒之因素，本活動一切更動將以衛生局官網公告為主，如有未盡事宜，並得隨時補充公告之。 ⭐參賽者如有偽造或不實者，經查證屬實，衛生局得依法追回其獎金及獎狀。 ☎️聯絡窗口：臺南市政府衛生局曾小姐 06-2679751#276 Tainan City's 2015 "Breakfast GO - Academic Achievement and Nutrition Double Bonus Campus Healthy Eating Reward Program" is now underway~~ Eligibility: Students currently enrolled in all levels of schools in Tainan (junior high school, vocational high school, college, and higher education). Event Period: Tuesday, August 19, 2015, to Friday, October 10, 2015 How to Participate ✅ During the event period, take a photo of your creative breakfast recipe and accompanying information. Eligible participants will receive a chance to enter a lucky draw. ✅ Participants can choose the category they wish to submit: (1) Chinese breakfast, (2) Western breakfast, (3) convenience store breakfast, (4) Tainan snacks. The proportion of dishes in the uploaded photos must follow the National Health Administration's "My Plate". You can refer to the Ministry of Health and Welfare's "My Plate" balanced diet images and tips for your own combination (URL: https://www.hpa.gov.tw/Pages/List.aspx?nodeid=4686) ✅ You can upload multiple times (different meal photos), but each person can only have a maximum of 5 lucky draw opportunities, and each person is limited to 1 prize. ✅ Those who upload meals and text descriptions that meet any two of the following principles are eligible for the lucky draw: (1) My Plate Principle (2) Reduce sugar, (3) Reduce salt, (4) Intake of whole grains. ✅ During the event, nutritionists will screen and evaluate the submitted photos to see if they meet the requirements. The lucky draw will be held after the event ends on October 10. 📌Designated website for event uploads: https://p.tainan.gov.tw//UhacZ2 📌Lottery period: Drawings will be made before October 31st, and winners will be announced on the Bureau's official website. Event Rewards: 🎁Early Bird Health Award: Submissions will be accepted from August 19th to August 31st, 2015. The first round of draws will be held. 30 winners will each receive a NT$200 gift certificate (limit one per person). The prizes will be announced on the Bureau's official website before September 30th. 🎁Healthy Living Award: Submissions will be accepted until October 10th. All qualified participants will be drawn for 50 winners each to receive a NT$200 gift certificate (can be combined with the Early Bird Award). 🎁Healthy Breakfast Endorsement Award: 20 winners will be drawn to receive NT$800 gift certificates and a "Best Healthy Breakfast Endorsement" certificate if the same participant submits at least once in all four categories and passes the evaluation by the Department's nutritionists. !!! Notes ⭐ Participants must be current students at junior high schools and above in the city and provide authentic information for notification of winners. Participants under the age of 20 should provide their legal representative's information on a Google form. ⭐ Prizes will be mailed after the event. Winners must complete and sign the collection form before they can receive their gift certificates. ⭐ The organizer reserves the right to modify the event format, rules, and prizes. In the event of force majeure, all changes to this event will be announced on the Department of Health's official website. Additional announcements may be made at any time for any unspecified matters. ⭐ If any participant falsifies or misrepresents information, the Department of Health may recover the prize money and certificate upon verification.
