一、 依據教育部115年1月29日臺教學(五)字第1152800451號函及本校「特定人員尿液篩檢及輔導作業要點」規定:每學期開學日起三週內經導師、校內之任課教師、專業輔導人員或職員工觀察後,依特定人員類別提報特定人員名冊,並交由學務處承辦人彙整,並召開會議審查。
二、請各班導師(含系主任)於114-2學期開學日起三週內【115年3月13日(五)下班前】,觀察班上/週遭學生其生理及生活狀態是否符合:「有事實足認為有施用毒品嫌疑之學生。」若有則請登入網路或掃描QR code(如附件)詳細填單回傳,如班上均無符合者,亦請務必於三週時限內(3月13日前)完成回傳,未回傳者視同無特定人員提列。
三、另學期間若發現學生施用或持有不明藥物,或觀察生理或生活狀態,認有可疑為施用或持有毒品之情形,亦請致電校內分機2202、2203洽生輔組承辦人關長順先生協處。

1. In accordance with the Ministry of Education letter dated January 29, 2026 (Ref. No. Tai-Jiao-Xue (V) 1152800451) and the University’s Guidelines for Urine Screening and Counseling of Designated Personnel, within three weeks from the start of each semester, class advisors, on-campus course instructors, professional counselors, or staff members shall conduct observations and, based on the categories of designated personnel, submit a roster of designated individuals. The roster shall then be compiled by the responsible officer of the Office of Student Affairs and reviewed through a convened meeting.
2.All class advisors (including department chairs) are requested to complete the screening within three weeks from the start of the 2026 Spring Semester (114-2), no later than March 13, 2026 (Friday), before the end of the workday. Advisors should observe students in their classes and surroundings to determine whether their physical or daily conditions meet the criterion of “students for whom there are sufficient facts to suspect drug use.”
If any such cases are identified, please log in online or scan the QR code (as shown below) to complete and submit the form. If no students meet the criteria, submission is still required within the three-week period (by March 13). Failure to submit will be regarded as no designated personnel being reported.
3.If, during the semester, students are found to have used or possessed unknown substances, or if observations of their physical or daily conditions raise suspicion of drug use or possession, please contact the Student Assistance Section at extensions 2202 or 2203 and consult Mr. Chang-Shun Kuan for further assistance.
