中華民國諮商心理師公會全國聯合會欲進行「成人心理健康、社會情緒與求助行為調查」，敬請本系教職員生踴躍填寫。

說明：
一、本調查目的旨在了解成人對心理健康與社會情緒現況、心理健康求助態度與服務使用情形及人工智慧（AI）於心理健康領域之關聯等，做為相關推廣上的依據。
二、本問卷採匿名方式作答，所填寫的資料皆為保密，僅供做為整體的調查統計分析資料使用。
三、本調查提供禮卷，將於115年12月底前，隨機抽選200位願意參與抽獎活動之填答者，贈送100元的7-11禮卷以資感謝。

The National Federation of Counseling Psychologists Associations of the Republic of China is conducting a survey on "Adult Mental Health, Social Emotions, and Help-Seeking Behavior." Faculty and students of this department are encouraged to participate.

**Instructions:**
1. The purpose of this survey is to understand adults' current status regarding mental health and social emotions, their attitudes towards seeking help and their use of mental health services, and the relationship between artificial intelligence (AI) and mental health, providing a basis for related promotion.
2. This questionnaire is anonymous, and all information provided is confidential and will only be used for overall survey statistical analysis.
3. A gift certificate will be awarded. By the end of December 2026, 200 respondents who wish to participate in a lucky draw will be randomly selected to receive a NT$100 7-Eleven gift certificate as a token of appreciation.
