一、國立陽明交通大學執行旨揭計畫期望透過區域基地模式，健全跨校教師發展支持網路，提升教師教學研究能量和教學品質，建構跨校教師專業知能。為協助教師掌握教學實踐研究計畫撰寫脈絡，提供一對一諮詢服務，藉以增加計畫申請及通過率。

二、實施方式：書面諮詢、線上或實體一對一面談諮詢。

三、申請期間：115年10月19日(一)至11月27日(五)止。

四、申請資格：
(一)全國大專校院符合申請教學實踐研究計畫資格之教師。
(二)申請人須提出116年度教學實踐研究計畫申請。
(三)每位申請人諮詢服務至多以二次為限（包含向其他區域基地學校申請諮詢次數），且申請人應回傳第一次諮詢紀錄及回饋表後，始得申請第二次諮詢服務。
(四)優先錄取「本基地之跨校教師社群成員」、「桃竹苗區域大專校院（含國立臺灣大學）教師」。

五、申請流程：
(一)請於115年11月27日（五）前填寫申請表後簽名，上傳至Google表單https://forms.gle/McJUciQosjbZEGx29。
(二)本基地收到申請後協助媒合一對一諮詢，並通知申請結果。如順利媒合，申請表及計畫書初稿將提供顧問教師參考。

六、諮詢結束後一週內繳交諮詢紀錄表，寄至本案聯絡人信箱wanping@nycu.edu.tw。

七、諮詢期間：115年10月27日（二）至115年12月4日（五）。

I. The National Yang-Ming National Chiao Tung University's implementation plan aims to improve the cross-institutional faculty development support network through a regional base model, enhance faculty teaching and research capabilities and teaching quality, and build cross-institutional faculty professional knowledge. To assist faculty in mastering the writing process of teaching practice research projects, one-on-one consultation services are provided to increase the application and approval rates.

II. Implementation Methods: Written consultation, online or in-person one-on-one consultation.

III. Application Period: October 19th (Mon) to November 27th (Fri), 2026.

IV. Eligibility:

(I) Faculty members from universities and colleges nationwide who meet the eligibility requirements for the Teaching Practice Research Project.

(II) Applicants must submit a 2027 Teaching Practice Research Project application.

(III) Each applicant is limited to a maximum of two consultations (including consultations requested from other regional base schools), and applicants must submit the first consultation record and feedback form before applying for a second consultation. (IV) Priority will be given to "members of the cross-institutional faculty community of this base" and "faculty members of colleges and universities (including National Taiwan University) in the Taoyuan-Hsinchu-Miaoli area."

V. Application Process:

(I) Please complete and sign the application form by November 27, 2026 (Friday) and upload it to Google Forms: https://forms.gle/McJUciQosjbZEGx29.

(II) After receiving the application, this base will assist in matching applicants for one-on-one consultations and notify them of the application results. If a match is successfully made, the application form and a draft of the proposal will be provided to the consultant for reference.

VI. Within one week after the consultation, please submit the consultation record form to the contact person's email address: wanping@nycu.edu.tw.

VII. Consultation period: October 27, 2026 (Tue) to December 4, 2026 (Friday).
